In order to prevent misunderstandings regarding dental benefits, we would our patients to know that all professional services rendered are charged directly to the patient, and that the patients are personally responsible for payment of fees. All accounts are due and payable at time of services rendered, unless prior arrangements have been made. We do not render our services on the basis that insurance companies will pay our fees. As a courtesy we will process and submit any necessary insurance paperwork, but all unpaid balances left after insurance are the full responsibility of the patient.

For your convenience we accept Visa, Discover, American Express and MasterCard. Extended payment options are available with approved credit through outside agencies. If you have any problems or questions, please ask our staff. They are well-informed and up-to-date. Please call if you have any questions or concerns regarding your initial visit.

So that we may assure you and other patients of uninterrupted treatment, it is necessary for all patients to accept and adhere to a definite arrangement of appointments and fees. Once an arrangement is made, please remember this time is reserved for you. At least 48 hours notice must be given if cancellation is necessary, otherwise cancellation fees will be applied. For surgical appointments, we require 72 hours advanced notice of cancellation to avoid fees. Failure to notify us in a timely manner denies other patients the opportunity to complete their surgical care.
